*** Please mark on the memo line what you would like the donation to be used for OR communicate this via email to the BMP. Options for donation can be dental team, dental lab, BMP Fluoride Program, medical team, audiology team, Rice and Beans program, our scholarship program, general funds, or other. Any donation received that does not have this specified in the memo section of the check and/or has not been communicated to the BMP will be deposited to our BMP General Fund to be used for general needs of the entire organization.

All donations come with a reciept of your donation for you to use for your tax purposes.
